I would hope the U.S. Environmental Protection Agency would temporarily block all companies from contracts until they can prove they meet U.S. government business standards. Every company is subject to cutting corners and risk taking when under pressure to produce on time and under budget. But I don't see any way to effectively evaluate company ethics, the root cause of most of this type of problem.</p>
