OSHA Cites Metal Container Corp. Following Amputation Accidents

The Occupational Safety and Health Administration issued Metal Container Corp. one willful and 13 serious citations for alleged violations that resulted in several amputations.

OSHA initiated an inspection of Metal Container Corp., an Anheuser-Busch Inbev company, in October 2010 in response to a report of accidents at the facility in Arnold, Mo. One accident led to an employee losing fingers in machinery, while another resulted in a worker losing a foot in a forklift accident, the federal agency stated.

OSHA issued one willful citation for hazards related to unguarded machinery. A willful violation exists, according to OSHA, when an employer has demonstrated either an intentional disregard for the requirements of the law or plain indifference to employee safety and health.

The serious citations were issues for alleged violations associated with exits, flammable or combustible materials, personal protective equipment, lockout/tagout of energy sources, forklift use, machine guarding and electrical deficiencies.

Metal Container Corp. has 15 business days from receipt of the citations to comply, request an informal conference or contest the findings.
